    Impact and Cost-Effectiveness of Point-Of-Care CD4 Testing on the HIV Epidemic in South Africa.

    Rapid diagnostic tools have been shown to improve linkage of patients to care. In the context of infectious diseases, assessing the impact and cost-effectiveness of such tools at the population level, accounting for both direct and indirect effects, is key to informing adoption of these tools. Point-of-care (POC) CD4 testing has been shown to be highly effective in increasing the proportion of HIV positive patients who initiate ART. We assess the impact and cost-effectiveness of introducing POC CD4 testing at the population level in South Africa in a range of care contexts, using a dynamic compartmental model of HIV transmission, calibrated to the South African HIV epidemic. We performed a meta-analysis to quantify the differences between POC and laboratory CD4 testing on the proportion linking to care following CD4 testing. Cumulative infections averted and incremental cost-effectiveness ratios (ICERs) were estimated over one and three years. We estimated that POC CD4 testing introduced in the current South African care context can prevent 1.7% (95% CI: 0.4% - 4.3%) of new HIV infections over 1 year. In that context, POC CD4 testing was cost-effective 99.8% of the time after 1 year with a median estimated ICER of US$4,468/DALY averted. In healthcare contexts with expanded HIV testing and improved retention in care, POC CD4 testing only became cost-effective after 3 years. The results were similar when, in addition, ART was offered irrespective of CD4 count, and CD4 testing was used for clinical assessment. Our findings suggest that even if ART is expanded to all HIV positive individuals and HIV testing efforts are increased in the near future, POC CD4 testing is a cost-effective tool, even within a short time horizon. Our study also illustrates the importance of evaluating the potential impact of such diagnostic technologies at the population level, so that indirect benefits and costs can be incorporated into estimations of cost-effectiveness

    The dynamic nearest neighbor policy for the multi-vehicle pick-up and delivery problem

    In this paper, a dynamic nearest neighbor (DNN) policy is proposed for operating a fleet of vehicles to serve customers, who place calls in a Euclidean service area according to a Poisson process. Each vehicle serves one customer at a time, who has a distinct origin and destination independently and uniformly distributed within the service area. The new DNN policy is a refined version of the nearest neighbor (NN) policy that is well known to perform sub-optimally when the frequency of customer requests is high. The DNN policy maintains geographically closest customer-to-vehicle assignments, due to its ability to divert/re-assign vehicles that may be already en-route to pick up other customers, when another vehicle becomes available or a new customer call arrives. Two other pertinent issues addressed include: the pro-active deployment of the vehicles by anticipating in which regions of the service area future calls are more likely to arise; and, imposition of limits to avoid prohibitively long customer wait times. The paper also presents accurate approximations for all the policies compared. Extensive simulations, some of which are included herein, clearly show the DNN policy to be tangibly superior to the first-comefirst-served (FCFS) and NN policies

    Measles vaccination and antibody response in autism spectrum disorder

    OBJECTIVE: To test the hypothesis that measles vaccination was involved in the pathogenesis of autism spectrum disorders (ASD) as evidenced by signs of a persistent measles infection or abnormally persistent immune response shown by circulating measles virus or raised antibody titres in children with ASD who had been vaccinated against measles, mumps and rubella (MMR) compared with controls. DESIGN: Case-control study, community based. METHODS: A community sample of vaccinated children aged 10-12 years in the UK with ASD (n = 98) and two control groups of similar age, one with special educational needs but no ASD (n = 52) and one typically developing group (n = 90), were tested for measles virus and antibody response to measles in the serum. RESULTS: No difference was found between cases and controls for measles antibody response. There was no dose-response relationship between autism symptoms and antibody concentrations. Measles virus nucleic acid was amplified by reverse transcriptase-PCR in peripheral blood mononuclear cells from one patient with autism and two typically developing children. There was no evidence of a differential response to measles virus or the measles component of the MMR in children with ASD, with or without regression, and controls who had either one or two doses of MMR. Only one child from the control group had clinical symptoms of possible enterocolitis. CONCLUSION: No association between measles vaccination and ASD was shown

    LCOGT Network Observatory Operations

    We describe the operational capabilities of the Las Cumbres Observatory Global Telescope Network. We summarize our hardware and software for maintaining and monitoring network health. We focus on methodologies to utilize the automated system to monitor availability of sites, instruments and telescopes, to monitor performance, permit automatic recovery, and provide automatic error reporting. The same jTCS control system is used on telescopes of apertures 0.4m, 0.8m, 1m and 2m, and for multiple instruments on each. We describe our network operational model, including workloads, and illustrate our current tools, and operational performance indicators, including telemetry and metrics reporting from on-site reductions. The system was conceived and designed to establish effective, reliable autonomous operations, with automatic monitoring and recovery - minimizing human intervention while maintaining quality. We illustrate how far we have been able to achieve that.Comment: 13 pages, 9 figure

    Modeling the color evolution of luminous red galaxies - improvements with empirical stellar spectra

    Predicting the colors of Luminous Red Galaxies (LRGs) in the Sloan Digital Sky Survey (SDSS) has been a long-standing problem. The g,r,i colors of LRGs are inconsistent with stellar population models over the redshift range 0.1<z<0.7. The g-r colors in the models are on average redder than the data while the r-i colors in the models are bluer towards low redshift. Beyond redshift 0.4, the predicted r-i color becomes instead too red, while the predicted g-r agrees with the data. We provide a solution to this problem, through a combination of new astrophysics and a fundamental change to the stellar population modeling. We find that the use of the empirical library of Pickles (1998) instead of theoretical spectra modifies the predicted colors exactly in the way suggested by the data. The reason is a lower flux in the empirical libraries, with respect to the theoretical ones, in the wavelength range 5500-6500 AA. The discrepancy increases with decreasing effective temperature independently of gravity. This result has general implications for a variety of studies from globular clusters to high-redshift galaxies. The astrophysical part of our solution regards the composition of the stellar populations of these massive Luminous Red Galaxies. We find that on top of the previous effect one needs to consider a model in which ~3% of the stellar mass is in old metal-poor stars. Other solutions such as substantial blue Horizontal Branch at high metallicity or young stellar populations can be ruled out by the data. Our new model provides a better fit to the g-r and r-i colors of LRGs and gives new insight into the formation histories of these most massive galaxies. Our model will also improve the k- and evolutionary corrections for LRGs which are critical for fully exploiting present and future galaxy surveys.Comment: Submitted to ApJ Letters.     Towards an autonomous DataFactory for the small-batch cooling crystallisation of active pharmaceutical ingredients

    As a method of forming and purifying the pharmaceutically relevant polymorph[1], crystallisation of an active pharmaceutical ingredient (API) is a key step in pharmaceutical manufacturing. Determining an industrial-relevant approach for API crystallisation can be resource-intensive as a candidate crystallisation process is constrained by and assessed against industrial relevant solubilities, downstream processing practicalities, and regulatordetermined Critical Quality Attributes (CQA) of the API[2-4]. The DataFactory at the CMAC aims to use high throughput smallbatch cooling crystallisation experiments coupled with machine learning to reduce the time and material costs associated with this process. Alongside the development of automated data collection, we are incorporating an autonomous decision-making system to optimize the small-batch cooling crystallisation of APIs and calculate relevant kinetic parameters to inform larger-scale experiments. Here we present the steps we’re taking to integrate and automate different platforms via a cobot and a central control PC, in addition to the beginnings of the database that will be the foundation of a crystallisation classification system
